Athletic Registration

To all student athletes and parents it is now time to complete the online athletic registration on PlanetHS.  There are 8 forms that need to be completed.  Some of the forms need to be completed electronically online and others need to be upload to PlanetHS.  The forms that need to be completed online are:

              1.  Pre-participation Physical Evaluation Form

              2.  Consent and Release from Liability Certificate

              3.  Verification of Insurance Coverage Requirements

              4.   Middle School Sportsmanship Contract

These are the forms that need to be uploaded to PlanetHS:

              5.  Birth Certificate

              6.  Physical Examination  (EL2) 

              7.  Student Code of Conduct Contract

              8.  Covid-19 Consent Form

You can download all of the forms that need to uploaded to PlanetHS from www.ksmsathletics.com under the Links/Forms tab.  Parents make sure that the student is electronically signing each form that is submitted.
